Leadership and clinical roles for licensed professionals, care managers, and specialists who coordinate and oversee client care.
Client Care Manager (CCM)
Lead comprehensive care coordination for older adults, working directly with families and healthcare teams to develop and manage personalized care plans.
Essential Functions
- Conduct initial in-home assessments and ongoing care evaluations
- Coordinate with physicians, specialists, and community resources
- Serve as the primary point of contact for families and caregivers
- Develop, implement, and update individualized care plans
- Travel to client homes and care facilities within the Bay Area
Qualifications
- Bachelor's degree in social work, gerontology, nursing, or related field
- Minimum 2 years of experience in care management or aging services
Physical Requirements: Ability to travel to client locations and navigate home environments including stairs. May occasionally lift or carry materials up to 25 lbs, with or without reasonable accommodation.
Registered Nurse (RN)
Provide clinical nursing expertise within our geriatric care management team, supporting health assessments, medication reviews, and care transitions for older adults.
Essential Functions
- Perform in-home health assessments and cognitive screenings
- Review medications and coordinate with physicians on care plans
- Educate families on health conditions, treatments, and care strategies
- Monitor client health status and escalate concerns as needed
- Travel to client homes and healthcare facilities within the Bay Area
Qualifications
- Active California RN license in good standing
- Minimum 2 years of clinical nursing experience, geriatric or home health preferred
- Current BLS/CPR certification
Physical Requirements: Ability to travel to client locations, perform physical assessments, and navigate home environments including stairs. May frequently lift up to 25 lbs and occasionally up to 35 lbs during patient care tasks, with or without reasonable accommodation. Assistive devices are provided for patient handling tasks exceeding 35 lbs.
Care Manager
Coordinate and oversee care services for aging clients, ensuring high-quality support that aligns with each individual's goals, preferences, and evolving needs.
Essential Functions
- Develop and implement individualized care strategies
- Monitor care quality and adjust plans as client needs evolve
- Facilitate communication between families, providers, and caregivers
- Conduct regular check-ins with clients and family members
- Travel to client homes and partner facilities within the Bay Area
Qualifications
- Bachelor's degree in social work, gerontology, healthcare administration, or related field
- Minimum 1 year of experience in care coordination or case management
Physical Requirements: Ability to travel to client locations and navigate home environments including stairs. May occasionally lift or carry materials up to 25 lbs, with or without reasonable accommodation.
Social Worker (LCSW / MSW)
Provide psychosocial support, crisis intervention, and resource navigation for older adults and their families during challenging care transitions and complex family dynamics.
Essential Functions
- Assess emotional, social, and environmental needs of clients and families
- Connect families with community resources, benefits, and support programs
- Facilitate family meetings, mediate care decisions, and provide counseling
- Develop safety plans and intervene during crisis situations
- Travel to client homes and community settings within the Bay Area
Qualifications
- Master's degree in Social Work (MSW) required; LCSW preferred
- Minimum 2 years of experience with geriatric populations or healthcare social work
